Ban Appeal Form

If you were banned by an admin or moderator, you will have to make an appeal on the forum. This thread will show you how to that.

The Form:
When making an appeal, use the following form:

If you do not use the correct format to make your appeal, your appeal will be disregarded by default. So use it!
Also note that if you lie on your appeal, by for example saying you were banned without reason, you will be asked to make another, honest appeal.

After sending in your appeal, the moderator responsible for your account being banned will answer your post, with either:
"ACCEPTED"
or
"DENIED"
deciding when and if you are to be unbanned.
Also know that only the moderator that banned you will be the one to unban you, and only him. No mod, and I mean NON of them will unban your account except for the one responsible.

Also please know that if you decide to keep making appeals because you were not happy with the decision made, your useless appeals will be deleted, and you will be given an infraction, the size depending on how many excess appeals you made.
